Disease Prevention
Description: Team approach to treatment of chronic pain with both pharmacological and non-pharmacological interventions with a focus on reduction of opioids.
Aim: Assist patients with chronic pain issues to have a better quality of life.
community group education programs
Health Promotion
Description: A series of group education sessions that are open to the general public, held throughout the year at various facilities (see events for schedule of classes).
Aim: : To improve health literacy and provide single session group presentations on a variety of health topics for HFHT patients and the population in surrounding communities.
take control, take charge
Health Promotion
Description: Once a week for six weeks - self-management program for patients with chronic pain or chronic disease (each held once or twice a year – dependant on demand).
Aim: To assist patients living with chronic pain or chronic disease better manage their symptoms and improve their quality of life.
