Adult Immunization Program 

Disease Prevention

Description: To ensure all adults received the appropriate immunizations (as per Ontario Immunization Guidelines). 
Aim: Disease prevention and to advocate/champion immunization. Increase patient’s knowledge so that they are aware of risks and are able to make an informed decision.

ADHD Nurse Practitioner Program 

Health Promotion

Description: To provide assessment of patients with a differential diagnosis of potential ADHD, assistance with management of patients with ADHD and management of patients transitioning from childhood treatment to adult treatment.
Aim: To provide education, assessment and treatment to patients with ADHD.

Bone Health Program

Disease Prevention

Description: To screen for occurrence osteoporosis by regular screening, preventative care and health education (as per osteoporosis guidelines).
Aim: To increase patient knowledge and awareness of bone health, and increase early detection and decrease the incidence of osteoporosis by regular screening, preventative care and health education/promotion.

Breast Feeding Support Program

Health Promotion

Description:  Provide education, encouragement and support for breast feeding. 
Aim: The promotion of breast feeding and for HFHT to eventually achieve Breast Feeding Initiative status.

Breast Screening Program 

Disease Prevention

Description: To provide screening for development of breast cancer (as per CCO guidelines).
Aim: To increase occurrence of early detection of breast cancer by regular screening and health promotion and education.

Cervical Cancer Screening Program 

Disease Prevention

Description: To screen for development of cervical cancer (as per CCO guidelines).
Aim: To increase occurrence of early detection and decrease the incidence of cervical cancer by regular screening, preventative care and sexual health education.

CHANGE Program

Health Promotion

Description: 1-year evidence-based diet and exercise intervention for people with metabolic syndrome (start date Sept 2019 – only available in Dunnville at present).
Aim: To improve health and decrease the risk of developing chronic disease in patients with Metabolic Syndrome through a lifestyle intervention model that provides the effective support needed to change lifestyle over time and produce a lasting improvement to overall health.

Childhood Growth and Development Program

Health Promotion

Description: To monitor for healthy growth and development in childhood. 
Aim:  Identify deviation from norm and refer for timely assessment/treatment. Increase parental knowledge, provide support and promote healthy, active lifestyles.

Childhood Immunization Program

Colorectal Screening Program 

Disease Prevention

Description: To screen for development colon cancer (as per CCO guidelines).
Aim: To increase occurrence of early detection and decrease incidence of colon cancer by regular screening, health promotion and health education.

Diabetes Management Program 

Chronic Disease

Description: To support best practices health management for patients with or at risk of developing DMII.
Aim: In conjunction with Community Diabetes Programs to improve monitoring and maintenance of blood sugar and lipids while ensuring that patients receive evidence-based screening and care for diabetes.

Health Heart (Lipid) Group Education 

Health Promotion

Description: To encourage self-management, preventative care, healthy lifestyles and to provide health education on various topics related to heart health and cholesterol.
Aim: To improve knowledge and support patients in making healthy lifestyle changes to reduce their CVD risk.

Post-Partum Depression Monitoring Program 

Health Promotion

Description: To screen for development of postpartum depression (PPD) from delivery to 1-year PP (as per Canadian OBGYN guidelines).
Aim: Identify deviation from norm and refer for timely assessment/treatment. Provide education and support.

Prenatal Program

Health Promotion

Description: Provide assessment, care and education throughout pre-conception, pregnancy and postpartum period.
Aim: Promotion of healthy outcomes during pre-conception, pregnancy and postpartum period

RN assisted OTN Program (in progress)

Health Promotion

Description: To facilitating audio-video consultations between various specialist and patients.
Aim: : To decrease wait time for specialist consultation and increase patient uptake of OTN.

Take Control, Take Charge

Health Promotion

Description: Once a week for six weeks - self-management program for patients with chronic pain or chronic disease (each held once or twice a year)
Aim: To assist patients living with chronic pain or chronic disease better manage their symptoms and improve their quality of life.
